Children must provide proof of identity with a birth certificate showing the names of their parents. If both parents are living, they both must sign the passport. Sole surviving parents and guardians must provide an affidavit stating they are the sole guardian. The form should be provided to the Passport Office.
Each applicant must provide two identical photos. The photos should be signed by the person applying for the passport and a member of the Garda Siochana. Photos should be less than six months old. They can be color or black and white. The photo should be a head shot. Sunglasses aren't allowed and subjects shouldn't show a police or military uniform in the photo. Application forms (APS 1) are available at every Garda station and at post offices that show the "Service Plus" logo. Once the forms are filled out, they must be witnessed by a member of Garda Siochana, as well as the two passport photographs. The fees required, proof of Irish citizenship and the most recent passport must be included with the application form. If using the Passport Express service, there is a special envelope that should be used to send in the form. It has a checklist of all the necessary documentation that is required. The application form, envelope and documentation can be handed in to the postal clerk along with payment.
