Visit the San Francisco Office of Vital Records, located at 101 Grove Street, Room 105. The office is open Monday through Friday from 8 a.m. to 4 p.m.
Fill out a birth certificate application form. You will need to include your mailing address, as well as birth information for you or the registrant.
Present the birth certificate application form---along with a valid photo ID and any necessary authorization---to a staff member.
Sign a sworn statement in the presence of a staff member. By signing this statement, you affirm that you are eligible to receive a certified birth certificate copy.
Pay the application fee of $14 per birth certificate. This is payable by credit card, money order or cash.
Download a birth certificate application form from the San Francisco Office of Vital Records website. Fill out the application form completely and accurately. Do not sign the sworn statement.
Take the sworn statement to a notary public. Sign it in her presence. The Office of Vital Records will reject your application if the sworn statement is not notarized.
Include any necessary documentation.
Write a money order for $14 per birth certificate made payable to SFDPH. You may also pay by credit card by filling out the included payment form. Cut it out and attach it to your application page.
Include a self-addressed, stamped envelope. If you wish to expedite your order, include a self-addressed, prepaid UPS, FedEx or USPS envelope and add a $15 expedite fee. Mail the package to: SFDPH OVR, 101 Grove St., San Francisco, CA 94102.
