A Canadian adult living in Canada, the United States or Bermuda must complete the PPTC 153 form to apply for a Canadian passport. The application form for Canadian children living in these locations is the PPTC 155. These forms are submitted along with all of the required supporting documentation outlined in the application form. The PPTC 054 form is for renewal of an existing passport. You may apply for a passport or a renewal by mail or in person at a Passport Canada service location, a participating Canadian post office or a participating Service Canada center. In the United States, applications may be made in person by visiting a Canadian government office.
The cost for a 24-page passport for Canadian adults living in Canada is $87 CA, and $97 CA for Canadian adults living in the United States or Bermuda. The cost for a 24-page Canadian passport is $37 CA if the child is 3 to 15 years old or $22 CA if the child is under 3 years old. The 48-page Canadian passport is $39 CA for children aged 3 to 15 years and $24 if the child is under 3 years of age.
For Canadians living outside of Canada, the United States or Bermuda, the correct application form is the PPTC 040 for adults and the PPTC 042 for children. The PPTC 054 form is used for renewal of an existing passport. Applications must be submitted to the nearest Canadian government office that provides passport services, including Canadian embassies, Canadian high commissions and Canadian consulates, except those headed by honorary consuls.

The PPTC 190 application form is required for non-Canadian adults living in Canada, and the PPTC 192 form is for non-Canadian children. After Passport Canada receives a form and supporting documentation, it determines whether a Travel Document or a Certificate of Identity is required and how long the document is valid.
A Travel Document applies to people with protected person status, including convention refugees and people in need of protection. It is endorsed for travel to any country except the country of citizenship.
A Certificate of Identity is for individuals who have been legally landed in Canada for fewer than three years, who are stateless or who are unable to obtain a national passport for a valid reason. It is not valid for travel to the bearer's country of citizenship.
The application fee for a non-Canadian adult is $87 CA. If Passport Canada determines that a Travel Document is required, no additional fees apply. If it determines that a Certificate of Identity is required, an additional fee of $40 CA must be paid before the application is processed. The fee for non-Canadian children age 3 to 15 years is $37 CA, and it is $22 CA for non-Canadian children under 3 years of age.
Application forms are available on the Passport Canada website, but they also can be obtained in person at any Passport Canada office, Canada Post outlet or Service Canada center.
If you were born outside of Canada to a Canadian parent between Feb. 15, 1977, and April 16, 1981, you also may be required to fill out the PPTC 001 -- Proof of Canadian Citizenship form.
A PPTC 203 form must be filled out to make a statutory declaration reporting a lost, stolen, damaged, destroyed or inaccessible Canadian passport or travel document.
