Gather your documents. You will need a copy of your National Statistics Office (NSO) birth certificate on security paper and a criminal record clearance from the National Bureau of Investigation (NBI). Passport photographs are taken at the Department of Foreign Affairs at the time you make your application.
Fill out the application. Passport applications are available on the Department of Foreign Affairs website at Dfa.gov.ph. Applications must be filled out completely.
Apply in person at the Department of Foreign Affairs. Arrive as early as possible to avoid long waits.
Gather your documents. If you are an active duty military person traveling on a military ID only, you must get a certification issued by your commanding officer that states you are still a Filipino citizen who is on active duty in the United States. You will also need a copy of your NSO birth certificate on security paper, and copies of any marriage certificates, school records and other supporting documents that verify your citizenship. Passport photographs are taken at the consulate at the time you make your application.
