Look for your previously issued passport. You will need to reapply if it has expired and you were younger than 16 when it was issued; if it has expired and it has been more than 15 years since it was issued; or your name has changed. You will also need to reapply if it was lost or stolen. If you previously applied for a passport but were rejected, gather all the documents in your application package.
Get Form DS-11: Application for a U.S. Passport. You can download it by following the link in Resources or get one at the post office. Fill out the application completely. Do not leave empty boxes. Write "N/A" if something is not applicable. Sign and date the DS-11. If you previously applied for a passport with a DS-11 but were rejected, make sure you have filled out everything correctly.
Verify your proof of citizenship. You can only apply for a U.S. passport if you are a U.S. citizen. You must submit an original birth certificate, consular report of birth abroad, naturalization certificate, or previously issued (undamaged) passport. If you previously applied but did not provide one of these original documents, you must do so when you reapply.
Get valid photo identification. This must be presented when you reapply for a passport. The ID can be valid driver's license, government ID, military ID, or previously issued (undamaged) passport. You cannot reapply for a passport without valid photo ID.
Take regulation passport photographs. They must be taken within six months of reapplying for a passport. The photos need to be identical, in color, and measure 2 inches by 2 inches. They must also show your full face, without any coverings or hats. The only exception is prescription glasses that you wear all the time. It is best to take your passport photographs at an establishment that specializes in them to ensure that they meet regulations. Bad photographs are a common reason for application rejections.
Pay the proper fee. Reapplying for a passport costs $100 as of 2009. This must be in the form of a check or money order to the U.S. Department of State. Applications are rejected if the fee is not paid in full.
Go to a passport office or acceptance agent with all of your passport application materials. (You can find links to lists of both in References.) Make sure all your documentation is in order before you go. Submit your application in person. Your new passport should arrive within a month.
