How Can I Request a Blue Card by the Mail?

A blue card in most states is commonly known as a handgun license or registration card. To be eligible for a blue card, applicants must be at least 21 years of age or older, pass a state-required background check and a gun-safety exam. Besides your application, you will need to submit a copy of your state identification card, your Social Security card and a recent photo of your face and head. Although you will have to apply in person to receive your initial blue card, you can request your replacement by mail.

      Visit the official Utah Department of Safety website link and click the "Forms & Applications" link located on the left side of the webpage, and then scroll down to the "Concealed Firearms" section and click the "Application for Concealed Firearm Permit" link.

      Print the application and complete it with all required information such as your first and last name, home and work number, date of birth, current mailing address and your Social Security number. Be sure to have your signature notarized on the application before submitting it.

      Include and submit with your application the following information: a 2-by-2 passport quality picture, a photocopy of your driver's license and one "Blue and White" fingerprint card. Include a check, money order for $65.25 dollars or enter your Visa or MasterCard credit card number on the application along with your 3-digit number on the back of the card and the expiration date.

      Enclose all required documentation and your application in a 9-by-12 envelope and send it to:
