Complete Part A of the GST form. This includes your name, business name, address, city, state, country, zip code and phone number. Choose your language. Complete Section A with your visiting information, and whether this is an amendment to a previous application.
Complete Part B (reason for rebate) and check either foreign representative or diplomat. After this section, you must total up all your expenditures eligible for GST rebate. Prepare all your receipts.
Calculate all eligible expenditures for GST/HST. Groceries, drugs, medical services and farm-related products are rated at zero. The exempt items include housing, education, child care and other daycare services. GST/HST rebates are for business supplies and services. You can add greater detail for these purchases in Part F, Page 3. Please note that if you plan to import or export these supplies from your home country, additional rules apply.
Complete Part C of the application with the totals you calculated on Page 2. Parts A, B, C and E are required to legally complete the application. Parts F and G are elective.
Attach receipts or certified copies of receipts. Section II and Part D are elective parts of the application. Please note that most receipts will list the GST tax paid on the receipt. There's no need to guess the amount of GST paid on a purchase.
Complete Section E, and certify the application with the date and your signature. Mail your application to the address listed in the top right-hand corner. Parts F and G are elective parts of the application to provide additional information on your business.
