Visit your local Department of Motor vehicles to pick up an application for disabled parking. Alternatively, many states have a DMV website where you can download an application that can either be mailed in or delivered to your local office.
Fill out the application; indicate whether you need a temporary handicap permit or a permanent one.
Have your doctor verify your need for handicapped parking space by signing the application. Fully complete all portions of the application along with any third party signatures that may be required.
Have the signatures on the application notarized if required by law. The application should state if this is a requirement.
Return the application to your local DMV and receive your permit allowing you to park in a handicapped parking space.
Pay any required fee related to your permit. In many states there is no charge but some do require a minimal fee. This should be stated on the application.
