How to Get a Passport for a Baby With Only One Parent Available

When applying for a passport for a minor child -- any child younger than 16 -- both parents must appear in person at a passport acceptance facility with the child. Unless a court order states that the child is under the custody of only one parent, both parents' signatures are needed on the application. If one parent cannot attend the appointment, the other parent must provide a notarized statement from the absent parent granting permission for the child to apply for a U.S. passport.

      Complete form DS-11 for your child. This is the application for a U.S. passport. You can obtain and fill it out at the passport acceptance facility during your passport appointment, or you can download and print it prior to your appointment.

      Visit your local passport acceptance facility with proof of your child's citizenship, proof of your relationship to your child and your photo identification.

      Present the passport application, your supporting documents and your spouse's notarized document granting permission to the agent at the passport acceptance facility.

      Allow the passport agency to take your child's photo for the passport. Photos must meet all U.S. Department of State requirements or the application will be denied. Photos must be 2 inches by 2 inches, against a light-colored background, and show just the child's face wearing nothing on his head or face except prescription glasses.

      Sign the passport application for your child and pay the required fee ($105 as of 2011). To expedite the handling of your child's passport and receive it in 2 to 3 weeks rather than 6 to 8 weeks, include an additional check for $60 to pay for expedited service.
