How do I Get Passports for Children?

The U.S. Department of State requires that all minor children under age 16, including infants, have their own passports when traveling abroad by air. Travel documentation requirements for minors traveling by land or sea are outlined by the U.S. Customs and Border Protection. Minors age 16 and 17 must meet special passport requirements as outlined by the U.S. State Department. Consider applying for a passport for your child as soon as possible to prevent any difficulties or delays if you need to travel abroad.
  1. Passport Application Process

    • Complete Form DS-11 Application for a U.S. Passport on behalf of your minor child and submit it at a passport agency or acceptance facility -- designated court houses and post offices, along with fees, photographs and all required documentation. Required documentation includes evidence of citizenship, evidence of the relationship between the minor and the parent or guardian, proof of identification of the parent or guardian and any necessary written parental consent. Parents or guardians and the minor must appear in person to submit the application.

    Evidence of Citizenship

    • Minors applying for passports must submit one of the following as evidence of U.S. citizenship: certified birth certificate issued by the city, state or country where the minor was born, naturalization certificate, certificate of citizenship, consular report of birth abroad or certification of birth.

    Evidence of Relationship Between Child and Parent or Guardian

    • Minors applying for passports must submit one of the following as evidence of the relationship between the parent or guardian and the minor: court order establishing guardianship or custody, minor's certified U.S. or foreign birth certificate indicating both parents' names, minor's report of birth abroad indicating both parents' names or an adoption decree indicating adopting parents' names.

    Proof of Parent or Guardian Identification

    • Both parents or guardians of minors applying for passports must submit a photocopy of one of the following as proof of identification: previously issued and undamaged U.S. passport, valid driver's license, current military or government identification or a naturalization certificate. Photocopies must be on 8 1/2-inch by 11-inch white paper, show both the front and back of the identification, be free of other marks and be printed on one side only.

    Additional Parental Consent

    • Form DS-11 must be signed in front of the passport acceptance agent by the minor and both parents or guardians at the time of application. If only one parent or guardian is appearing in person with the minor, a notarized statement of consent from the second parent or guardian is also required. If only one parent or guardian with legal custody is with the minor, the parent or guardian must also submit one of the following as primary evidence of sole authority to apply for a passport of the minor: consular report or certification of birth abroad, minor's certified U.S. or foreign birth certificate listing only the parent or guardian who is applying, an adoption decree or a court order for sole custody to the parent or guardian making the application.

    Photographs and Fees

    • Two 2-inch by 2-inch, full facial, identical color photographs of the minor taken within the last six months must be submitted at the time of passport application. Hats are not allowed. Prescription glasses normally worn should be worn for the photo. Passport fees must be paid at the time of application.
