To apply for a passport for your baby, you will need an application form. You can download and print one online or get one from your local passport acceptance facility. These can be found in some post offices, libraries, courts or government offices. You can fill out the application beforehand. However, you must wait to sign it in front of an Acceptance Agent. Your baby needs to appear in person and all guardians must be present as well.
Bring two passport photos of your baby. Bring her birth certificate, proof of U.S. citizenship and identification for both the guardians or parents. If one of the parents or guardians can't be present, bring a notarized statement of consent. If one of them is deceased or you are the only guardian, you must bring documentation proving your circumstance. You will also need evidence of the relationship between the guardians and baby, such as the baby's birth certificate or adoption papers.
There are two types of passports available, passport books and passport cards. A passport card is required for land or sea travel between the U.S. and Canada, Mexico, the Caribbean and Bermuda. When traveling to other countries than those listed, or when traveling to any country by air, a passport book is needed.
The application fee for a passport for children younger than 16 is $80 for a passport book, $15 for a passport card and $95 for both. There is also a $25 execution fee. If you need your passport in less than two weeks, you will need to go to a regional passport center. Extra fees will apply. Your baby's passport will be valid for five years.
