Passport applications are available for download at the U.S. Department of State's website, or call 877-487-2778 (TDD/TTY: 888-874-7793). There are different applications to use depending on your status.
A passport application must be submitted in person if the applicant is under the age of 16, or if the applicant is over the age of 16 and has never had a passport before. An the application must also be submitted in person if the applicant is over the age of 16 and their previous passport was lost, stolen, damaged, issued more than 15 years ago, or they cannot legally document a change of name. A person can renew their passport by mail if they are over 16 and their previous passport is undamaged, or was issued within the last 15 years in the applicant's current name (or if the applicant can legally document a name change).
The upper-right corner of each application contains the application's expiration date. A new application must be used if it is past the expiration date. There is also an estimated time given for how long it should take to complete the application.
The requested information is essential in processing the application, and falls into two categories: contact information (a mailing address where the applicant can receive the passport), and citizenship information. As stated on the application, knowingly providing false information on a passport application can result in a fine and/or imprisonment. Applications also clearly state "Incomplete or unacceptable applications will delay the issuance of your passport." It is essential to provide all the information requested.
