Use the Passport Application Wizard. This wizard is located on the Department of State website under the Passport link. Simply open the wizard and answer the questions, and the wizard will let you know what form (DS-11, DS-82, DS-5504 or DS-4085) you need to complete. The wizard will also estimate the payment you must make and generate the form you need.
Complete the form. If you are using Form DS-11, you can complete it online and print it ready to be submitted. The other forms need to be printed and then completed in blue or black ink.
Print the form correctly. A passport application form must be printed on one side of the paper only. Do not submit a form printed on both sides, or it may not be accepted. Follow the rules, and you will receive your approved passport much faster.
Compile the passport packet. Once you have completed your form, you need to compile the passport photos, appropriate fee, birth certificate, marriage certificate and any other information that is required. Once you have this information, take your packet to the passport acceptance facility (post office) to take the oath and sign your passport application. Then you can mail the completed application from there. If you are renewing your passport you will not have to do this, but you will need to send your expired passport in with your new application.
Check your passport status online. The Department of State also has a link on its website that allows you to check the status of your passport. You can find out if it is being processed, if it has been approved and mailed back to you or if there is a problem.
