On the following web page choose the "submit" button under "Apply Online" to be directed to the online application. Print the form when completed. Remember to sign and date the form.
Bring the required forms and documents, you passport photo and your soon-to-expire passport to the post office. Be on time for your appointment. Submit the documents to the passport official.
