How to Get an Italian Visa Application Form

Before you travel to Italy for extended business, education or tourist visits, you will need to obtain an appropriate visa to enter the country. A visa is a document that is stamped into your passport and grants permission to enter a foreign country for a set amount of time for a specific purpose. An Italian visa application can be submitted to any of Italy's embassies or consulates in the United States.

      Locate the closest Italian embassy or consulate to you by using an online locator tool (see resources). Click on the link to the right of the embassy or consulate name to view the office's website.

      Click on the "Forms" link at the top of the web page. Some embassy or consulate websites may list available forms under the "Visa" section. Click on the "Schengen Visa Form" link. The link may also be called "Application Form for Schengen Visas" or "Application Form."

      Print the blank application form or save the PDF document to your computer. The same application form is used for business, tourist and study visas.

      Obtain a blank Italian visa application by visiting an Italian consulate or embassy in person. Locate the office's address by visiting your local embassy or consulate's home page. The websites of Italian embassies and consulates list the address on the main page next to a photo of the office building.
