Types of Birth Certificates in Puerto Rico

As a United States territory, Puerto Rico issues the same types of birth certificates and requires the same paperwork as the other U.S. states and territories. Birth certificates can be obtained from a Puerto Rican vital records office by mail, phone or online. Since Puerto Rico designates access to birth records as "restricted," only the person listed on the certificate or that person's legal guardians can obtain an official version.
  1. Photocopy of Birth Record

    • This is the full, official birth certificate. It is the hardest to obtain and can be used in all cases when you need to prove your identity such as applying for a passport or registering for school. It includes information including the child and parents' personal information, the delivering doctors and signatures from the parents and doctors.

    Certified Copy from Computerized Abstract

    • This is also known as a short form birth certificate, and is a computer certification that proves the original vital record exists. It does not provide as extensive information as the longer version, but includes vital information such as the child's name, gender, birthplace and date of birth. It can usually be used in lieu of the longer version as a proof of identity.

    Birth Card or Wallet-Sized Birth Certificate

    • This is a wallet-sized version of the computer certification abstract.

    Commemor­ative Birth Certificate

    • This is either handed out by the hospital or can be ordered from the territory for keepsake purposes. It may have the baby's footprints on it, decorative fonts or gold embossing. It is not a valid form of identification, but can be framed as a family heirloom.
