How to Locate a Birth Record in Puerto Rico

The government of Puerto Rico has enacted measures to prevent the fraudelent use of birth records. All birth records issued before July 1, 2010 will be invalidated in favor of a new, secure birth record. If you were born in Puerto Rico and are currently residing outside of the Commonwealth, you may procure the new birth record by mail on or after July 1, 2010.

Instructions

      Download a copy of the "Birth Certificate Application by Mail," available through the Puerto Rico Federal Affairs Administration's website (see Resources). Fill out this form completely. If you are an immediate relative, heir or legal representative of the registrant, indicate your relationship to the registrant.

      Make a photocopy of your driver's license, state I.D. or passport. The Vital Statistics Record Office may accept other forms of government-issued photo I.D. at their discretion.

      Write a money order for $5 made payable to the Secretary of the Treasury. Additional birth certificate copies cost $4 each. If you are over 60 years of age or a veteran, you need not pay this fee.

      Enclose a self-addressed stamped envelope.

      Mail your application package to: "Puerto Rico Vital Statistics Record Office, P.O. Box 11854, San Juan, PR 00910."
