How to Get a Duplicate Permit if It Was Stolen or Lost in Pennsylvania

Proper identification is a necessity if you want to prove who you are to pick up a package, purchase alcohol or gain access to a club or bar. A government-issued photo identification -- such as a non-driver ID card, permit or license -- should always be carried with you. If you misplace your Pennsylvania permit or it gets stolen, you need to apply for a replacement permit with the Pennsylvania Department of Transportation.

      Print out a non-commercial driver's license application for change/correction/replacement.

      Complete sections A and B. Be sure to check the box next to "Lost" or "Stolen" to inform the Department of Transportation why you're submitting the application.

      Skip section C unless you need to make a change or correction. If you're under 18, ask your parent or guardian to fill out section D.

      Fill out section E and then take the form to a notary and sign in section F. Many libraries and banks have notaries available, or you can go to your local town hall to sign the form. Bring an alternate form of photo identification, as it will be required by the notary.

      68272, Harrisburg, PA 17106-8272.

      Include a check or money order made out to PennDOT for $13.50 to cover the cost of replacement.
