- Native American tribal document with tribal seal and photo
- Valid Canadian driver’s license or provincial ID card
- Canadian provincial health card with photo

- Two recent (taken within the past 6 months) professional quality photos
- Passport photo size (2 x 2 inches)
- Color, front view of applicant against a white background
- Clear and sharp
- No facial hair or eyeglasses/sunglasses
- Neutral facial expression (don’t smile)
- Shoulders must be visible

- Parents or legal guardians listed on the application must appear with child to present their valid ID at time of application
- Parents or legal guardians must submit a consent form if application is being filed for international travel without one or both parents/legal guardians

- If previous U.S. Passport was lost or stolen a form DS-64.
- If applicant’s name has been changed from that of his or her birth certificate a legal name change document issued by a federal or state court.
- If name on the birth certificate is different than the name currently being used supporting documentation such as a marriage license, divorce decree or court order.
