How to Order an Official Birth Record From Jackson County, Missouri

To register a child for school or apply for a passport, a copy of a birth certificate is typically a required form of identification. The Jackson County Health Department can provide you with copies of birth certificates for births that occurred anywhere in the state since 1920. You must be the person listed on the birth certificate (registrant) or the registrant's relative, other than a cousin, to obtain the birth certificate copy.

      Download an Application for Copy of Birth or Death Certification from the Missouri Department of Health and Senior Services website (see Resources).

      Complete the "Birth" section of the application, providing the registrant's name, date of birth, place of birth and parents' names. Provide your name, address, the reason for the request and your relationship to the registrant in the bottom section of the application.

      Mail or bring your application and a check or money order for the application fee, which is $15 as of November 2010, to the Jackson County Health Department (see Resources). Include a self-addressed envelope if submitting by mail. The Jackson County Health Department fulfills walk-in requests the same day.
