Complete and print the necessary passport application forms. In the U.S., these are the application form DS-11, and the Lost or Stolen Passport Statement, DS-64. DS-11 and DS-64 can be downloaded at the Lost and Stolen Passports section of the Department of State's Travel website. If you do not have access to a printer, you can request the forms at the embassy or passport facility.
Submit the forms, along with two passport photographs and travel information, in person at a passport agency or acceptance facility. Provide evidence, such as an airline ticket, that you require a passport within 24 hours. This service is approximately $170, including the $60 expediting charge, plus overnight courier fees.
Visit a legitimate online passport agency. No matter what company you choose, make sure to research its background and business practices before committing to it. These companies will act as liaison between you and the state department.
Call the company and speak to an agent, either to make an appointment or to coordinate mailing and delivery times.
Complete and print the DS-11 and DS-64 forms, which are available on any passport agency or state department website.
Send the completed DS-11 and DS-64 forms, along with two passport pictures, travel information and fees, by Fedex or courier to the company address. Some companies also require authorization letters, which legally entitle them to act on your behalf. Once your application is successfully completed, the company will send your new passport by overnight mail.
