Fill in the appropriate passport form. A new passport form will ask for personal details such as where you were born and your parents names. There is usually a space to add your parents passport numbers, which can speed up processing for your application. A renewal form will ask for some personal details, along with your old passport number, and a space to declare any change in circumstances. Make sure you use black ink and it is legible.
Visit the passport office if you filled in DS-11. A passport official will read you a declaration, and you sign your passport in front of them. Do not sign your name before this. If you have filled in a DS-82, you can mail this back to your passport office. Enclose your old passport in the envelope, along with a check for the renewal fee and photographs. Send it certified mail to ensure it gets there.
Pay your fee at the passport office. The fee itself will vary, and you may be offered a faster service for a higher fee. You may also need to display identification documents such as a birth or marriage certificate, and you will need two color passport photographs. The official should enclose these documents with your application.
