To apply for a passport, you will need proof of U.S. citizenship. Such proof includes a formally-issued passport, birth certificate, Consular Report of Birth Abroad or Certification of Birth, Naturalization Certificate or Certificate of Citizenship. An early public record, such as a hospital birth certificate or school record may also suffice as proof of citizenship. In addition to the teen's proof of citizenship, parents must also submit evidence of relationship, which includes a birth certificate or adoption records with the names of the parents on it. Parents must also present their own government-issued identification. Parents of minors 16 and older need not present identification if the teenager has their own.
Teens and parents will need to complete and submit form DS-11: Application for a U.S. Passport. You may not sign until in the presence of Acceptance Agent with the minor and both parents.
Teenagers under the age of 16 must pay $105 in passport fees for a passport. Those who wish to obtain a passport card in addition to a passport book must pay $120. Teenagers 16 and older must pay $110 for a passport book and $140 for a passport book and card.
You must apply in person if you are under the age of 18 and present identification or your parents' identification. If your passport was lost, stolen or damaged you will also need to appear in person to apply for a new one. Teenagers must also establish parental consent, which may be a written and signed document stating that they can obtain a passport.
