Visit an acceptance facility in the state of Wisconsin to apply for a passport in person. To search for a passport acceptance facility in your city, go to the U.S. Department of State website's Acceptance Facility Search Page (see Resources).
Obtain your the DS-11 Passport Application from the acceptance facility or at the Department of State website.
Fill out the application online, or print out a blank copy and complete it manually. You must include your social security number; your application cannot be processed without it. Do not sign the application until you are at the acceptance facility and the agent tells you to do so.
Go to the acceptance facility with your completed application, proof of citizenship and identity, and photocopy of your identification.
Acceptable citizenship documents include a previously issued U.S. passport, U.S. birth certificate, consular report of birth abroad, naturalization certificate or certificate of citizenship.
Proper identification includes a previously issued passport, naturalization certificate, valid driver's license, current government ID or military ID.
The photocopy of your identification should be on an 8.5-by-11-inch sheet of white paper, and must show both the front and the back of your identification. The sheet may have no other images or markings.
Give the agent proof of your U.S. citizenship and your identification.
Pay the application fee of $140 for both the passport book and card plus $25 for the execution fee. You may also incur additional fees if you need the passport expedited quickly. Some acceptance facilities take cash but most facilities accept personal checks or money orders. Passport agencies, however, will accept payment with credit cards.
Present the acceptance agent with two identical passport photographs. The photos must have been taken within the last six months, and must show you wearing ordinary street clothes.
