Log on to the U.S. Department of State's "Passport Application: DS 11" web page. Complete form DS 11 online and print it, or print the form and fill it out by hand.
Find the nearest passport acceptance facility. Kansas City has a number of acceptance facilities, including the Jackson County Court of Appeals, the North Kansas City Post Office and the Civic Center. Log on to the U.S. Department of State's Passport Acceptance Facility Search Page and type in your zip code to find the facilities nearest you. Contact the facilities to find out their regular business hours.
Take the completed DS 11 form, along with your proof of citizenship, such as a birth certificate, previously issued passport, naturalization certificate or certificate of citizenship; a valid piece of identification, such as a driver's license, government or military i.d., or naturalization certificate; your two passport photos; and the first-time applicant fee ($135 as of 2010) to the nearest passport acceptance facility in Kansas City.
