Complete passport application form DS-11 by hand, using a black ink pen. Do not use a computer to fill out the application and do not sign it yet. You will be asked to sign it at the consulate.
Write a statement describing how and when your passport was damaged. This statement can be typed on a computer or handwritten, but it should be hand-signed in black ink. If you are filling out the application for a child under the age of 16, proceed to Step 3. If you are filling it out for yourself and you are older than 16, skip to Step 4.
Fill out the statement of consent for issuing a passport to minors, but do not sign it. (See Resources 3) The statement of consent must be signed and dated in front of a certified notary public and submitted with the DS-11 application at the consulate. You must be the parent or legal guardian of the minor applicant.
Get your passport photos. Passport photos should be 2 inches by 2 inches, on a white background, with no hats or facial coverings and the photos should be identical. Photos can be taken by a passport photo photographer, or can be printed on your computer, as long as glossy photo paper is used to print the photos. Photos printed on copy paper will not be accepted.
Gather evidence of US citizenship. You must be able to prove that you are a citizen of the United States via birth certificate, naturalization documents or proof of immigration. This can be a little bit of a challenge while in Australia, since you will need an official copy from your birth state. A copy will not suffice. If you have the original at home, have a trusted friend or family member FedEx the original document to you. If you don't have the original, order one from the records department in your birth state, have it mailed to your home and then have a friend ship it to you.
Gather proof of identity. A driver's license, foreign passport, military or other government-issued, photo identification is acceptable.
Make copies of all forms and supporting documentation. Consulate employees will look over the original documentation to ensure its authenticity, and will take copies of the originals to refer to. If you do not have access to a copy machine, the consulate can make copies of the documents for you for $1 (USD) each document.
Using Express Post or Express Post Platinum, fill out a self-addressed envelope so that the consulate can return your new and old passports. Registered Post envelopes are not accepted.
Prepare your fees. As of 2010, passport fees for in-person applications begin at $105 (USD). For oath services the fee is $50 (USD). Cash fees are accepted in exact dollar amount, notes only and in US or Australian currency. Money orders and bank checks are acceptable and should be made out to "U.S. Consulate General." Payment via credit card is acceptable and must be conducted in person.
