Fill out Form DS-11: Application for a U.S. Passport completely and thoroughly. Do not sign the form until you are told to do so by a passport acceptance agent at the USPS facility.
Locate your proof of citizenship. You must have an original or certified birth certificate, a previously issued passport, a certification of birth abroad, a naturalization certificate or a certificate of citizenship.
Determine what form of identification you will use. You must present a valid driver's license, government or military identification card, previous passport, or naturalization certificate. It must have your legal name, birth date, your photo and a date of issuance. You must also provide a copy of the front and back of this identification card on a white 8 1/2 inches by 11 inches sheet of paper.
Purchase two identical 2 inch by 2 inch passport photos at your local pharmacy store or photography shop. You may also choose to have these taken at the acceptance facility when you turn in your passport documents and application. You should wear normal attire but do not wear anything on your head. If you wear glasses, these should be worn in your photo.
Take all of your documents and application to the USPS passport acceptance facility. Once the agent certifies your documents and application, she will ask you to sign the application. Pay the passport fees. As of December 2010, there is a $25 processing fee and a $110 fee for the passport.
