Download the passport application Form DS-11, which you can find on the United States Department of State's website at Travel.state.gov. Fill out the form online before printing it, or print a blank one and fill it out by hand. Don't sign the form until instructed to do so by the application acceptance facility.
Go to your nearest passport acceptance facility. Look for the nearest passport acceptance facility using the online search system on the United States Department of State's website at Iafdb.travel.state.gov.
Bring evidence that you are a U.S. citizen in the form of a birth certificate, an old, undamaged U.S. passport or a naturalization certificate.
Identify yourself with photo identification such as an old U.S. passport, a valid government or military ID, a driver's license or a naturalization certificate.
Provide photocopies of the front and back of your identifying documents. Use 8 1/2 x 11 inch, standard, white paper stock for your copies. Don't reduce the size of the images. Increase the images for legibility purposes if needed.
Supply two color, passport photos that are no older than six months and 2 x 2 inches in size.
Pay the expedited service fee as well as the passport processing fee. As of December 2010, this expedited fee is $60, the application fee is $110 and the execution fee is $25.
Receive your passport in about two to three weeks.
