Complete the passport application form. The form can be found at the website of any Philippines consulate general or embassy. For links to all consulates, visit the website of the Embassy in Washington D.C. (see References). The application form is a one-page document requesting personal information with space for right and left ink thumbprints. Do not make the thumbprints ahead of time; they must be made in the presence of a consular officer.
Gather the supporting documents, which include: current passport (if applicable); original and photocopy of proof of immigration status (green card, citizenship certificate, visa); three identical 4.5-by-3.5 cm color photos set against a royal blue background (make sure to tell the photographer the exact specifications); proof of Philippines citizenship (passport, birth certificate, naturalization certificate or Report of Birth document); and photocopy of passport information page (for those who previously held a Philippines passport).
Apply in person to one of the eight consulates in the U.S. or one of the two U.S. territories (Saipan and Guam). The consulate generals in the United States are in San Francisco, Los Angeles, New York, Chicago and Honolulu with honorary consulates in Atlanta, Miami and Portland (Oregon). You must take originals of all required documents with you to the application interview.
Pay the $50 application fee at the time of your interview. You will also submit ink thumbprints in the presence of a consular officer. If you do not intend to return to the consulate to retrieve your passport, submit a prepaid and self-addressed envelope along with your application material so that your passport can be mailed to you.
