Instructions

    • 1

Get an application form. You can download one from the Passport Canada website or use the Passport On-line system if you wish to do it by computer. If you would rather deal with a person, you can pick up the application at a Passport Service Centre, a Canada Post location or a Service Canada Centre.

    • 2

Fill out all the relevant sections of the form, and assemble the necessary paperwork. This includes your proof of Canadian citizenship, at least one identity document and an accepted form of payment for your passport fee.

    • 3

Take two identical passport photos of the correct size to attach to your application. The photos must have been taken within the past 12 months.

    • 4

Have your guarantor fill out the Declaration of Guarantor section of your application, and sign the back of one of your photos after writing, "I certify this to be a true likeness of (your name)."

    • 5

Submit your forms by mail, at a Service Canada office, a participating Canada Post office or a Passport Canada office.
