Online applications replaced paper visa forms as of May 24, 2010. Have on hand your passport, and any other documents relevant to your application, along with an electronic copy of a photo of the applicant to upload.

Review the information you entered into the online visa application. Print the confirmation page and keep it in a safe place because you must take the confirmation page with you through all the remaining steps of the visa application process, according to the Bureau of Consular Affairs.
Schedule an appointment at the U.S. embassy by calling 1-909-101-7878 or visiting the website philippines.us-visaservices.com/Forms/DetermineTCN.aspx. You must enter the DS-160 confirmation number into the appointment system to schedule an interview.
Pay the applicable visa fee at any designated Citibank or BDO locations. You must pay the application fee before your appointment.
Take your DS-160 confirmation page, your passport and two passport-size photos with you to your appointment. The photos are required despite the fact that you uploaded an electronic photo with the online application. The U.S. Embassy address is 1201 Roxas Blvd., Ermita 1000, Manilla, Philippines.
