Check that you are eligible for the Freedom Pass. If you are aged 60 or over, you automatically qualify. However, if you are applying for one based upon a disability, you must prove that the disability is valid. To do this, visit: http://www.freedompass.org/disabledusers.htm.
Once you are sure that you are eligible, visit http://www.freedompass.org/ where you will find a link to apply for the pass. There are different forms to fill depending on whether you are applying for a Freedom pass for a disability or due to age. If you are disabled, you must contact your local council before you can apply; members can tell you everything you need to know for your application.
After clicking on "Apply for a Freedom Pass," you will be taken to a page where you scroll down to find a link to the application form. You can download this form or pick up an application form from your local post office.
Once you have filled in the forms, take them to your local post office along with proof of identity, age and address. These proofs can come from documents including a current passport, a letter of your pension entitlement, a medical or a birth certificate, provided your name hasn't been changed. As proof of address you can show a recent utility bill, a current television license or a council tax letter or bill.
You also will need two passport-sized photos of yourself as further proof of identification. Once your documents, your local post office will give you the pass.
